I don't know if I'm smart or not, but I think part of the joke is that the husband doesn't recognize that his now-wife is his former best friend, and that unlikely circumstance kept me from understanding the joke even though I read the explanation. (Not the explanation in this thread but the more ambiguous one in the other thread.)

I'm generally aligned with the "let people enjoy things" view. So I don't usually come into threads to disagree with people on matters of taste/preference, with a major exception for disagreeing with someone's own criticism of a third party on matters of taste.
OP came in an posted a joke you have to work for. Someone else came in and complained about it. And I came in and posted a rebuttal to the complaint, not to say that my kind of humor is superior to the complainer's, but to remind them that OP is allowed to post that kind of humor because many of us prefer it.

As someone who transitioned and then dated her male best friend. It was fun, and while it didn't work out in the end because of things outside the relationship, it was one of the best relationships I've had.

Wrong again. It looked the same in 2021 when this image was already circulating.
It's a single frame from a panning shot where the actress is nodding her head quickly, and there is some out of focus camera work with lighting combining to make it look a little odd when you pause it right there. If you watch the clip you can see that moment.
Sometimes people just look weird if you take a frame out of a video.
