I've learned on the Sailor Moon subreddit that these long transformations, introductions, and yelling out your attack name and your own name that you often see depicted in anime actually stems from samurai culture. Samurais would have people helping them change for a fight right before the fight (transformation) and it was considered manners to introduce yourself at length to your opponent, so that if they brought your head home, they could say who it was whom they defeated - and vice versa.
Considering the power rangers are somewhat american and japanese and produced by toei, it would align with that idea.

)What happens if one samurai (A) defeats another samurai (B) who carries the head of a recently-defeated third samurai (C). Does B relay C's story to A? What if A also gets killed? Do they write this down?

Well, I am no expert in samuraiology, but I can very well imagine something like "I am Bankotsu, wrath of the Eastern lands, destroyer of flesh and reason, fear me and my banyru for its blade is sharper than the devil's sword, I have killed many great fiends for see I carry the head of Jakostu the gazing snake warrior, and I will soon be carrying your head home to my great master"
Basically casually dropping C's name. Like, that's what I'd do if I were B. (Except I wouldn't die of course as I would be the strongest and greatest warrior to ever live and I will definitely destroy my opponent.)
Edit: for the scenario of A and B dying I'd assume the villagers or accompanying vassals would be able to tell the tale and who's who. If everyone gets annihilated by an ancient nuke like attack then, well, bad luck I guess.

They also always wait when they defeat the villain to give them a chance to become a giant so they can bring out the megazord
